The way we work ensures that personalisation is at the very core of the Wellbeing Teams approach, and that the people are co-producing the support they receive. Our partnership with Community Circles helps people to stay connected to those around them, participating in local community services and even develops social capital. All of these aspects are mentioned in key policy documents, including the Care Act and the NHS 5 Year Forward View.
